No mobile information or other contact information will be shared with third parties/affiliates for marketing/promotional purposes. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.  

Informed Consent 

Kent County’s Ready by Five Millage is providing funding for the services you are receiving. By providing your consent, you understand the following about services that are funded with Ready by Five Early Childhood Millage funds:  

MomsBloom is required to collect information about you and your child. Your information will be shared with the Kent County Health Department, Kent ISD and First Steps Kent for the purposes of community reporting, program improvement and evaluation.  

All reports made publicly available to the community by MomsBloom, Kent County Health Department and First Steps Kent will not name you or your child nor include any information that would allow people to determine that you or your child received a millage-funded service.  

The type of information that MomsBloom will be collecting for Ready by Five Millage reporting includes:  

• For child and parent/caregiver information including name, address, birth date, gender identity, race/ethnicity, health insurance type, WIC enrollment, education, employment status, marital status, involvement of welfare services (if applicable), household income, and primary language spoken.  

• Information on services received such as date, location, type of service, child development screening result received, and satisfaction with services.  

• Information on referrals to services and resources and if your family received requested service or resources.  

• Home Visiting or Healthy Development Serving Expectant and New Parents Programming: Information voluntarily reported about expectant parent’s pregnancy and baby including due date and voluntarily self-reported health information about access to prenatal care, tobacco use, if breastfeeding, non prescription drug use, and concerns with post-partum well-being.  

• Information voluntarily reported by the primary caretaker about the child including information such as status of child’s immunizations, involvement of welfare services (if applicable), child development screening, and information on related referrals (if applicable).  

You may cancel this consent form at any time by sending a cancellation request in writing to MomsBloom at info@momsbloom.org. A more specific description of how your information will be shared and used for the purposes of Ready by Five Early Childhood Millage can be found as part of the Ready by Five Privacy Notice on the First Steps Kent website at www.firststepskent.org/readybyfiveprivacy 

 MomsBloom Client & Volunteer Agreement 

MomsBloom, Inc. ("MomsBloom"), is a non-profit organization that arranges for volunteers to provide assistance to families in their home. MomsBloom volunteers are not certified or licensed professionals. To help assure your best experience with our program, we do professional background checks and a required training of all our volunteers in an effort to provide your family helpful in-home support at no cost.  

By signing this release and waiver, I am requesting in-home assistance from MomsBloom and agree to the following:  

MomsBloom volunteers may not perform any medical tasks (including taking temperatures, administering medicine, etc.) or make any medical diagnoses.  

MomsBloom volunteers may not be left alone in the home to babysit. There will be at least one adult family member in the home at all times that a MomsBloom volunteer is in the home. 

I agree to waive, release, hold harmless, indemnify, and forever discharge MomsBloom and its affiliates, officers, directors, agents, members, employees, volunteers, successors, and assigns from all actions, causes of action, claims, demands, damages, and liabilities of any kind and nature, known or unknown, which I now have or might have in the future, relating in any way to the services I receive from MomsBloom.   

In the event that any claim is brought against MomsBloom (and/or its officers, directors, employees, or volunteers) relating to any services provided by MomsBloom to me and my family/household, I agree to indemnify and hold MomsBloom (and/or its officers, directors, employees, or volunteers) harmless for any such claim to the fullest extent allowed by law.  

My signature is a waiver and release on behalf of all members of my family/ household, including any children in my family/household, and otherwise binds all members of my family/household to this agreement.  

This release and waiver has been freely and voluntarily signed after careful review of all its terms and provisions. This waiver may not be rescinded, nullified, or canceled.  

Covid-19 Provisions, Release and Waiver.  I acknowledge that allowing a MomsBloom volunteer to provide in-home services in my house involves certain risks with respect to exposure to or contraction of the novel coronavirus, which has been declared a worldwide pandemic by the World Health Organization. Coronavirus is extremely contagious and believed to be spread by person to person contact.  I accept and voluntarily incur all risks of exposure, contraction, sickness or other damages related to coronavirus that may result from participation in MomsBloom’s services and elect to participate in spite of these risks.  I agree to follow any guidelines and instructions as required by governmental authorities or MomsBloom personnel, including, but not limited to, requirements of social distancing, pre-screening questionnaires or use of masks or other personal protection equipment in my acceptance of MomsBloom services. All such requirements, if any, will be communicated to me in advance of my first volunteer visit.   

Accordingly, I, for valuable consideration (including, but not limited to, MomsBloom permitting me to participate in its services), agree to waive, release, hold harmless, indemnify, and forever discharge MomsBloom and its affiliates, officers, directors, agents, members, employees, volunteers, successors, and assigns from all actions, causes of action, claims, demands, damages, and liabilities of any kind and nature, known or unknown, which I now have or might have in the future, relating to COVID-19. 

SMS Terms & Conditions 

1. SMS Consent Communication: 

The phone numbers collected during the SMS consent process will not be shared with third parties for marketing purposes. 

2. Types of SMS Communications: 

If you have consented to receive conversational text messages from MomsBloom, you may receive conversational SMS messages related to the following:  

  •  Appointment reminders 

  • Visit check-ins and/or Resource options  

  • Appointment scheduling  

3. Message Frequency: 

Message frequency may vary depending on the type of communication. Depending on your engagement, you may receive up to 2-3  messages per month. 

4. Potential Fees for SMS Messaging: 

Please note that standard message and data rates may apply, depending on your carrier’s pricing plan. These fees may differ if the message is sent domestically or internationally.  

5. Opt-In Method: 

You may opt in to receive SMS messages from MomsBloom in the following way:  

"By checking this box, I consent to receive Conversational SMS from MomsBloom. Reply STOP to opt-out; Reply HELP for support; Message & data rates may apply; Messaging frequency may vary. Visit https://momsbloom.org/privacy-policy  to see our privacy policy and Terms of Service.” 

6. Opt-Out Method: 

You can opt out of receiving SMS messages at any time. To do so, simply reply "STOP" to any SMS you receive. Alternatively, you can contact us directly to request removal from our messaging list.
